📈 Quality & Safety

W3.CSS
Quality & Safety accelerators are ideal for specific conditions or care settings where you want to measure and improve performance against a defined care pathway.

The Congestive Heart Failure (CHF) accelerator gives insight to inpatient CHF exacerbations. It provides a health network with a broad picture of the management of the heart CHF population when admitted to inpatient services. Including insights into how well patients are receiving care for acute inpatient admissions for CHF exacerbations, Diuretic Utilization, Weight Management, Patient Education, Outpatient Follow Up Appointments, Length of Stay, Readmissions, Mortality, and Utilization of Hospital Services.

Process, Medication, Length of Stay, Mortality and Readmission measures help an organization realize if clinical interventions and pathways are helping to improve the overall care of the CHF patient population. Appropriate patient education and fluid management are paramount to keeping the CHF population healthy.

📋 Appropriate Utilization

W3.CSS
Appropriate Utilization accelerators are designed to implement best practices based ordering and administration of labs, imaging, medications and other high-cost/high-risk resources.

Prevent Pain and Organisms From Skin And Catheter Entry And Radiology. The POKE-R program provides insight that ultimately can reduce the occurrence of adverse events, reduce waste during hospital admissions, and assist with decision support for making future procedure and medication choices. This program is proven to have significant cost savings.

Our configurable solution help organizations decide and customize events based on what is considered meaningful, invasive, and painful. Aligns with performance improvement initiatives, thus improving patient experience. POKE-R analytics, when embedded within EHR, provides flexibility to physicians, specialists and nurses - prior to and during rounds - on optimizing clinical care.

This Vancomycin accelerator tracks vanco days, creatine levels and pharmacy similar to the more expanded. Vancomycin Management Program inclusive of:

  • Pharmacy intervention
  • Kidney function
  • Therapeutic medication levels

Opioid is a high risk pain management program, classified as an epidemic by CDC. Effective prescription and administration of Narcan and Opioids are required in both an inpatient and ambulatory setting. Our utilization informatics provides ability to filter on:

  • DRG
  • Readmission
  • Length of stay
  • Mortality

Our Blood Utilization accelerator categorizes counts of blood administration units based on blood administration guidelines. Our solution categorizes blood in 5 distinct categories by provider, specialty, service, and department.

  • Category 1: Hgb less than 7 g/dL (Hct <21%)
  • Category 2: Evidence of acute bleeding with Hgb less
    than 8.5 g/dL
  • Category 3: Evidence of hemodynamic instability with
    Hgb less than 8.5 g/dL
  • Category 4: Hgb less than 8.5
  • Category 5: Any unit administered for an order that does
not meet criteria for any of the blood categories 1
through 4 will be in category 5. Category 5 will represent
the units administered with no documented justification.

Management of the central lines and foley catheters are important for patient safety. Our Lines, Drains & Airways accelerator includes informatics on:

  • How they are managed
  • Trending
  • Volume
  • Utilization by location
  • DRG

Our Ventilators accelerator tracks number of days on mechanical ventilation by location and service by DRG. Also tracks the number of patients on ventilators. Ventilator bundle performed- includes metrics such as:

  • Head of bed elevation
  • Daily sedation interruption
  • PUD prophylaxis
  • VTE prophylaxis

🏆 Operational Excellence

W3.CSS
Operational Excellence accelerators focus on streamlining clinical operations.

The LOS Accelerator provide hospital administration the opportunity to see how long on average their patients are admitted to their hospital and how they are performing against industry benchmarks. By having this data, they can strategically begin to identify bottlenecks in the process, such as prolonged admission and discharge times. Organizations also use the data to help improve patient satisfaction, decrease the opportunity for hospital acquired infections and reduce the overall cost per patient encounter.

The Readmissions metrics provide hospital administration the opportunity to see how often on average their patients are returning to their hospitals for readmission. Tracking this data provides the opportunity to identify where improvements can be made to decrease these rates. Reducing hospital readmissions in turn will lead to improved reimbursement by Medicare, better patient outcomes and discharge planning, and improved patient experience.

Measure and manage the ICU length of stay and case mix index. Cohort of encounters where the patient was admitted to the ICU at any point during the encounter.

  • Critical care readmission rates - cases in which the patients is transferred out of the ICU and later returns to the ICU

The Operating Room (OR) Efficiency metrics provide data for core performance indicators centered around performance and how well the operating rooms are being utilized throughout the organization. These OR specific metrics provide a detailed view on the overall hospital level down to individual operating rooms. This accelerator provide a way for hospital organizations to ensure hospital resources are being used efficiently and effectively. With these metrics, the organization can begin to benchmark and monitor the performance of the OR facilities. This data provides an insight into where revenue is potentially being lost or can be generated.
